P0138
O2 Sensor Circuit High Voltage (Bank 1 Sensor 2)
- Severity
- Low
- Driving
- Drivable — book it in
Oxygen sensor 2 on bank 1, downstream of the catalytic converter, is reporting persistently high voltage - a rich signal. As a monitoring sensor rather than a control sensor, this rarely changes how the car drives.
Possible causes
- CommonSensor signal wire shorted to a power supply
- CommonFailed oxygen sensor
- PossibleGenuinely rich mixture reaching the converter
- PossibleSensor contaminated by oil or coolant
What you might notice
- Check engine light on
- Usually no drivability change
- Failed emissions test
What to check, in order
- Check for upstream fuel or misfire codes first - a genuinely rich mixture will show up here too.
- Inspect the wiring for damage that could short the signal to power.
- Check the connector for water and corrosion.
- Replace the sensor once wiring and upstream mixture faults are ruled out.

Plug an ELM327 adapter into the OBD-II port under the dashboard. Pin16 reads every stored, pending and permanent code, explains each one like this page does — offline, with no account — and shows what the car recorded when the fault was set.
